Overdose Deaths in Washington Medicaid Enrollees on the Rise

Medicaid patients in Washington state made up nearly half of overdose deaths involving prescription opioids, with methadone as the leading drug implicated in those deaths, according to a report from state health agencies distributed by the CDC.
The analysis, which appears in the Oct. 30 issue of CDC’s Morbidity and Mortality Weekly Report, highlights the growing abuse of prescription painkillers, which fueled an 85 percent overall increase in poisoning deaths nationwide between 1999 and 2006, when 37,000 people perished.
During that same period, the use of prescription opioids quadrupled nationally, according to the report.
Washington has been among the nation’s leaders in these deaths, with a 2006 analysis finding that the state’s opioid overdose rate was 8.2 per 100,000, versus 4.6 per 100,000 nationally.
State health agencies decided to examine the prescription opioid abuse problem problem and found it was concentrated in the low-income Medicaid population.
From 2004 to 2007, Washington’s Medicaid population had an age-adjusted risk of death by prescription opioid overdose that was 5.7 times higher than the risk among the population as a whole: 30.8 persons per 100,000 among Medicaid recipients versus 4.0 per 100,000 in the general Washington population.
Researchers included in their findings all death certificates that included “poisoning by narcotics” or “mental and behavioral disorder due to use of opioids” and specific words compatible with acute drug intoxication in any of the cause of death fields, including specific prescription drug names, such as methadone and oxycodone.
Certificates listing morphine as a drug involved in the patient’s death — 82 in all, or 36.6% of deaths — were excluded due to potential confusion with deaths caused by heroin overdose. Morphine is a metabolite of heroin.
The study found 2,282 deaths met the manner and cause of death definition criteria. More than 96% of the certificates included an indication of acute drug intoxication, while 76% included an opioid term and were included in the analysis.
Among those included, 758 (45.4%) were enrolled in Medicaid at some point during their year of death.
The increased risk for those on Medicaid mirrored findings in an earlier study, which found heightened national risk for similar deaths in low-income populations, the Washington report noted.
Some estimates indicate opioid prescription rates for Medicaid users are at least double that of those with private insurance, the authors said.
In Washington, the government has established a Medicaid Patient Review and Coordination (PRC) program to deal with patients who misuse the system to get prescription painkillers — in particular by engaging in doctor or emergency room “shopping,” prescription forgery, and other abuses. Among PRC clients, the risk of death by prescription overdose was more than 15 times as high as among the general Medicaid population.
Among abused painkillers, the three most commonly in the Washington deaths were methadone (64%), oxycodone (22.9%), and hydrocodone (13.9%).
Methadone, in particular, has become a prominent drug in overdoses in the state study as well as nationally. The study attributed its popularity to its low cost and long period of action — it is also widely used in treating opioid dependence.
Overall, 72.3% of the Washington deaths had more than one drug listed on their certificates, and 54.6% of included at least one non-opioid drug: benzodiazepines were listed on 20.9% of death certificates and antidepressants on 31.7%.
An illegal drug was found in 21.8% of deaths, with cocaine (15.7%), methamphetamine (5.5%), and heroin (2.4%) as the most common. Alcohol was found on 17.1% of death certificates.
The study found most dead were male (58.9%), and a large majority (77%) were between the ages of 24 and 55.
The authors noted that the study was limited by potentially under-represented opioid deaths, such as cases where opioids were involved in mortality but not listed on death certificates, and suicides-by-overdose mislabeled as accidental deaths, or vice versa.

Stretch circularly and bring the arms ahead. Count to twelve and perform the reverse of the step. Circle the arms as largely as possible.
Step 5 to cure back pain
In an upright posture, widen the gaps between your left and right leg to a distance, place your left and right hand on your waist and gently rotate your waist in a clockwise and anti-clockwise direction up to twelve times.
Step 6 to cure back pain
In an upright posture, widen the gaps between your feet {left and right}. Place your hands tightly behind the head and try to bend the upper section of your body, {from the waist to neck region} in order to make your head touch your knee cap. Do these gently up to twelve times.
Step 7 to cure back pain
In an upright posture, slightly move your feet apart at a distance and bend the knees somewhat. Lift your arms to the height of your shoulders and lock your hands while turning to the side, starting at the waist region. Hold, count to six and do the reverse of the step. Then keep your hips and legs stationed as you turn the upper section of your body.It’s easy when you do it gently.
Step 8 to cure back pain
In an upright posture, while stretching the hands down to the sides. Bend the knees somewhat and gradually lift the left arm as far as you can reach over the head. Gently, glide the right arm, sliding it down to your feet, pull the left arm so that it is over the head as high as you can reach. Extend down to your thigh, returning to standing position and vice-versa.
Step 9 to cure back pain
In an upright posture, keeping the feet apart at a certain distance with your shoulders. Throw your left hand intermittently towards the direction of your right hand and throw your right hand intermittently towards the direction of your left hand as if you are punching on the boxing ring. Do these slow and steady for twelve times.
Step 10 to cure back pain
In an upright posture, locking your hands together and extending them behind your back. Lift your two hands up as high as you can. Counting to five times. Stand upright and keep the feet at the length of your shoulders. Bend your knees somewhat and tighten your fingers, while raising your hands to the height of your shoulders. Push the hands ahead in position. Do not lean to the front. Stretch and count to ten. Perform the same actions for up to five times.

Where possible you should avoid all refined carbohydrates including white breads and pastries!
If you aim is to reduce back pain through a balanced diet it’s obvious that you should try to eat a variety of mostly fresh foods with an emphasis on good old fashioned fruit, vegetables, nuts and grains with lean meats and fish being a great source . Unfortunately the snacking in between meals is detrimental unless it’s very, very healthy, and a common rule to stick to religiously is to never miss your breakfast.
An average person doesn’t eat a huge amount of food each day, so it is important that what you do consume is high in nutrition, and any supplements should ideally be consumed in their most natural form such as organic whole foods, as your body will be more capable of accepting them into its system.

Is Your Office Chair Being a Pain in the Back?

I have gone through a number of chairs over the years, as I ,usually am working very long hours at my desk, so comfort is critical for me and I’m sure you too. The comfort of my office has a lot to do with the comfort of my stool that I have to be on all day.
Since we sell office chairs, I am regularly asked by my store visitors about computer chair designs and what posture is the best for sitting when working long hours in a chair.
Some of these folks have been given very good quality high end ergonomic custom made chair at their office and are wondering if there is really any variation between it and your standard task chair. Some people complain about the shape of some guest or reception lounge seats at some offices, these persons often find themselves uncomfortable after having only sat for only a short stretch.
It is definitely true that some ergonomic chairs can feel considerably more comfortable than others. Some chairs make it a easy to maintain a improved upright posture, while others can get in the way between you and good comfort.
You need to bear in mind however, that it is our body that sits in a chair, and in order to increase on our own comfort, our interest should be focused on improving our own work place comfort and what we do with our body when we are in our chairs working hard.
So what advice can I give regarding posture and the everyday office chair? First, I would recommend very plain, basic type designs with fairly flat and firm seat bottom and with backs to not push your torso into any particular shape.
I would also suggest good back support and a decent tilt mechanism for proper reclining and movement while moving throughout the tasks of your day. But, you need to take into account that you cannot look to your chair as the primary solution to poor posture. It’s your posture that needs to realize how to change itself and find that comfort zone.
So a quality office seat can mean different things to different individuals, some might feel better even on a traditional type seat vs. a fantasy mesh chair. Other people may never find real comfort while sitting, and esp. larger people, or people with injuries or pain, for them this can all lead to increased exhaustion, pain and pain and discomfort, while sitting and working all day.
One of the most important things to seek out in a high-quality chair is the padding, this is something that the poor quality office chairs you often find at the chain stores are lacking in, the lower quality chairs are made with foam that might feel good when you test the chair out in the shop but it swiftly starts to breakdown and compress and lose the memory and after a few months it feels like you’re sitting on a stool with no padding once the foam becomes broken down.
The second most essential thing to look for in good quality office chairs is the seat mechanics, you want to make sure the chair is fully adjustable, a good chair will have about six to seven adjustments, where a typical low quality chair will have at best 3-4. Having those additional adjustments is very valuable to get an best working position. A quality office seat will adjust vertically as well as horizontally, you’ll also have seat incline, back angle, seat slide in and out away towards the back rest, and the back can adjust vertically. Speaking of seat dimensions, some chairs esp. the higher end chairs, will occasionally come with distinct size seat bottoms, small, medium and large seats. If you find that nearly all office chairs are either too small or too large then perhaps a specially sized chair give you the most comfort.

They compared the various treatments and options often available to whiplash/neck pain patients. They looked at 200 people suffering with chronic neck pain over 11 weeks of treatment, and recorded their progress over the next two years.
The patients were randomly divided to receive 20 one-hour treatments, in one of the methods listed below:
- Spinal manipulation and light soft-tissue massage from experienced chiropractic clinicians.
- Chiropractic spinal manipulation plus rehabilitative exercise from trained exercise therapists, including stretching and dynamic neck exercises.
- Rehabilitative neck exercises using a variable resistance, neck extension and rotation machine.
Patient-rated pain was lower for both exercise groups than for manipulation alone, and the ecercise groups benefited more regarding pain, disability, improvement and health status. Spinal manipulation plus exercise provided greater satisfaction than manipulation alone or rehabilitative exercises, however. The advantage of both manipulation plus exercise and machine exercises over mainpulation alone continued over the two-year follow-up period.
